The goals of our services is to:

  1. Strengthen and preserve the family unit
  2. Improve behaviors in the home
  3. Prevent out-of-home placements

Intensive-In-Home Services

Intensive in-home services are home-based mental health services that are designed to meet each child and family’s unique needs through crisis management, intensive case management, family therapy, counseling and practical skills training.  The goal of intensive in-home care is to reduce the use of residential placement and hospitalization by providing support in the home to keep families intact.  For those who have already entered treatment centers, intensive in-home services can help them transition back home quicker by providing patients and their families with the coping mechanisms, resources and tools necessary to successfully provide in-home care.

The target population for intensive in-home services are those ages five to 21 who live at home with their parents, relatives or legal guardians, or those in foster care.  To be eligible for Medicaid-funded intensive in-home services, individuals must also have received a clinical diagnosis due to severe mental, behavioral or emotional illness that significantly impair their ability to function normally within the community-at-large.

Intensive in-home services vary based on the individual need but may include:

  •  Anger management
  • Social coaching skills
  • Crisis management
  • Intensive case management
  • Counseling
  • Family therapy
  • Skills training

Mental Health Skill-Building

Our Mental Health Skill Building program consists of training and coaching that assists mentally-challenged individuals or those battling substance abuse with achieving and maintaining community stability.  This is done by teaching and reinforcing independent living skills, identifying and using other resources in the community, assisting with medication management and monitoring of psychical health and nutrition.

Skill building takes place in an individual’s home or community and varies based on the individual need of each patient.  Examples of mental health skill-building services may include:

  • Teaching laundry and housekeeping skills
  •  Providing budget management assistance
  • Teaching stress management and coping skills
  • Educating individuals on good personal hygiene
  • Assisting with medication management
  • Assisting with grocery shopping and meal planning
  • Symptom Management

Therapeutic Mentoring

Therapeutic Mentoring utilizes positive role models and community engagement to help at-risk youth function well in their school, social and daily living situations. The main goals of therapeutic mentoring are to:

  • Help clients improve their academic standing in areas such as reading, writing and mathematics
  • Encourage participation in sports, social and civic groups
  • Promote activities that connect clients with others in their community such as volunteering at the YMCA, local food pantry, shelters, etc.

Individuals who leverage therapeutic mentoring services may benefit from improved communication and interpersonal skills, learn to have healthy peer connections, positive, age-appropriate social interactions and enhance their ability for emotional expression.

Therapeutic mentoring takes place in an individual’s home or community and varies based on the needs of the individuals.  Examples of therapeutic mentoring services may include:

  • Coaching, supporting and training young adults in age-appropriate behaviors
  • Teaching interpersonal communication, conflict resolution and problem-solving skills
